データベース

　当院では、2004年より患者さんの診療データをデータベース化し、2021年度末で、約8,000症例のデータを蓄積しています。このデータベースを基に、当院ではComprehensive and Accessible Rehabilitation Databese（CARD）を開発しました。

　CARDは、データベースに蓄積された脳卒中患者さんの運動麻痺や筋力、関節の動く範囲などの運動機能、会話する・記憶する・問題を解決するなどの認知機能、歩く・着替える・体を洗うなどの日常生活動作能力の経過を即座にグラフ化できます（図1）。

　過去に入院されていた患者さんのデータから、症状が類似しているデータを抽出し、患者さんの治療計画や将来を予測する際の参考にしています。また、この経過は非常にわかりやすくグラフ化（図1）できるので、患者さん・ご家族さんへの説明にも活用しています。

　研究面では、リハビリテーションを365日実施することの効果や、リハビリテーション量を2時間/日から3時間/日に増やした場合の効果についていち早く検証し、公開をしてきました。

　継続して、改善を促進・阻害する要因は何か、どのような介入がより効果があるのか、より詳細に分析し、このビッグデータを患者さんのリハビリテーションに活用していきます。
